Audit evidence
Audit evidence is the set of records a provider can produce to show that a standard is met: policies with their history, acknowledgements, training and credential records, incident and complaint registers, and the actions taken on them.
Documents are not the same as evidence
A policy is a document. Evidence is that the policy was current on a given date, that the staff who needed it had acknowledged it, and that it was followed when it mattered. The difference is the record around the document: who, what, when, and whether anything has changed since.
What an evidence pack contains
Current policies and their version history, sign-off records, training status, credentials, and incident and complaint history with corrective actions, assembled for a period or a filter. Records that are timestamped and signed, with a way to verify that nothing has been altered, are the strongest form. What a particular audit will ask for is set by the regulator and the audit scope; confirm it with them.
