Feature · Incidents

Incident management, from the moment it happens to close-out.

A support worker types what happened, on a phone, straight after it happens. Accorda reads the account and proposes the title, type, severity and the policy it touches, each with its reasoning attached. A person reviews it before anything is recorded.

Accorda incident register showing reference, severity, status, owner, linked policy and reportable flags
A real screen from Accorda, not a mockup.

Capture in plain words

Incident forms fail when they ask a worker to classify before they describe. Accorda asks for the account first, in the worker's own words, and works out what it means: a suggested title, an incident type, a severity and the related policy, each with a short reason. Every suggestion is reviewed by a person and the original account is never altered.

Reportable incidents, corrective actions, closure

Decide whether an incident is reportable, assign corrective actions with owners, and track each one to close-out. The register shows reference, severity, status, owner and linked policy at a glance, so nothing sits open unnoticed.

  • Reportable flag on every incident, with the decision on the record
  • Corrective actions with owners, tracked through to closure
  • Each incident linked to the policy it relates to
  • Works on a phone in the browser; no app to install

Evidence that stands up

Every incident and every action is timestamped and SHA-256 signed. When an auditor asks for the incident register, Accorda generates it fresh from live records as part of the evidence pack, provably unaltered.

FAQ

Straight answers.

Does Accorda decide whether an incident is reportable?

No. It proposes a severity and type with its reasoning, and your team makes the decision. The decision and who made it are recorded.

Can staff log incidents from a phone?

Yes. Accorda runs in the browser on phones and tablets, so staff can log an incident wherever they are, straight after it happens.

How are corrective actions tracked?

Each action has an owner and a status, and stays open until it is closed. Internal audit findings become corrective actions in the same register.
