Policy acknowledgement
A policy acknowledgement, or sign-off, is a dated record that a named staff member has read and accepted a specific version of a policy, so a provider can show who had been told what, and when.
Why it matters
The expectation has shifted from "do you have a policy?" to "can you prove it was current, acknowledged and followed?" The acknowledgement is the middle term. Without it, a provider can show the policy existed but not that the worker involved in an incident had ever seen it.
What the record needs
The person, the policy, the version, and the date. When the policy changes, a fresh acknowledgement of the new version. A tick in a column cannot show which version was read or who made the tick, which is why a spreadsheet sign-off is the one an auditor tends to question.
